Returning Family & Student Re-enrollment Form 

This form is specifically designed for students who are currently enrolled in The Leadership and would like to enroll for the following school year. Enrollment in The Leadership Academy is reviewed on an annual basis. We aim to continuously foster growth in our students. When a student reaches a plateau we sit down as a family (administration, parents and/or student) and seek the optimal academic solution for our young leaders. 

New Family & Student Enrollment Form

This form is designed for the family who has been recommended, interviewed and is pleased with what The Leadership Academy offers. At The Leadership Academy we not only accept the child but the whole family. Completing this form and other enrollment requirements is a commitment to teamwork, our school vision and mission as well as growth and unity. Enrollment in The Leadership Academy is more than an avenue for education, it's a lifestyle! 

Withdrawal Form/School Transfer Form

This form is crafted to formally end an academic contract with The Leadership Academy. We thank each and every family that has put their trust in our school, our mission and team no matter the length of their tenure. We are always looking for ways to improve our offerings. With this form we look forward to your feedback and reflections on your time here with us as a family. Whether you are leaving for relocation, a student’s plateau, dissatisfaction or a better opportunity remember our saying “Once Leadership Academy [family], always Leadership Academy [family].”

Application for Payment Installment Plan

This application is for pre-approved returning or newly enrolled families in good financial standing with The Leadership Academy. Requests to enter individualized payment plan agreements, should be made to TLA Administration, via email. Please be aware that even after the application process, TLA Administration reserves the right to approve or deny the request, upon administrative discretion.
